How To Unlock iPhone:
Open iTunes on your Mac or PC and verify that you have internet connectivity.
If your phone is not the current version (most updated), it will prompt you to update. Update to current version.
Insert any micro-SIM card from supported networks.
It should automatically detect the new network. If it does not, then go to
Settings > General > Reset > Reset Network settings.
It will take about 30 seconds.
You will see the phone searching for signal. If it does not please do a 'Restore' and restart the phone.
It should now be unlocked permanently!

Just In Case You Have Issues:
If you are having issues to use the iPhone on different network, then there are the following things you can do.
* Insert a different network sim card, not the one original comes with the phone. Most of the time sim you are using is not good, it may be working just fine in your other phone but some phones does not accept a sim card and causing the problem.
a. Connect your iPhone to iTunes, wait until iTunes detects your iPhone.
b. If no messages comes up for unlock. restore and update via iTunes.

* Still have any issue then change the sim card and repeat the above process.

* Make sure you have not installed any other program after restore and update.

* Disable the 3g, as some network does not support 3g

* Call the customer support of the new network and see if the phone supported by the network.

* Reset the phone to factory setting and after that connect the phone to iTunes. Simply go to
Settings > General > Reset > Reset All Settings
Although by doing this all of the data on the iphone will be deleted so back up before doing this.

No Service With Any SIM
Probably you installed ultrasn0w on an incompatible baseband. Open Settings, click on General, then on About and scroll down to Modem Firmware to check the version. Ultrasn0w is compatible with these versions, depending on your device:
for iPhone 3G/3GS: baseband 5.13.04 or lower (or the iPad baseband 6.15.00)
for iPhone 4: baseband 1.59.00
for iPad: baseband 6.15.00

If you have any other baseband version, go to Cydia and uninstall ultrasn0w. If it still doesn't work, then you need to restore your device.
Another reason for this could be that your SIM is broken or has expired. Usually prepaid SIM cards expire after a few months when not in use. Test the SIM in another phone first.
A third reason could be a hardware fault. Most common is loose connection in the signal antenna cable. Contact to Apple service center to have it fixed.

* If your iPhone is not detecting the sim card or you are getting no service icon.
This is the most common problem when you have a phone that was or has been jailbroken. After you have factory restore, during activation, the phone does not detect the sim card and you get no service icon.
Here is what to do.
For jailbreak phones, using iPad baseband prevents the iPhone to update successfully. So you first have to downgrade your baseband while the phone is jailbreak. After that factory restore the phone and it will be factory unlocked.
DO NOT USE iPad BASEBAND WHILE UNLOCKING.
